Sagging Jowls in Seoul: Causes and Non-Surgical Options

Quick answer

Sagging jowls form when skin, fat and the deeper SMAS layer along the lower face lose support with age, so the jawline blurs and soft tissue gathers below it. At The Heal in Jamsil, Seoul, a dermatologist assesses which layers are involved, then recommends options such as Ultherapy Prime, Thermage FLX or combined lifting. Most have little downtime. Send front and side photos for a lifting plan before you fly.

What causes sagging jowls?

Jowls develop gradually as several layers of the lower face change at once:

  • Skin laxity: collagen and elastin decline, so skin stretches and loses snap-back
  • SMAS weakening: the deeper muscle-fascia layer that supports facial tissue loosens
  • Fat shifting: fat pads in the cheeks descend while volume is lost elsewhere, so weight collects along the jawline
  • Bone and ligament changes: the jaw and supporting ligaments change with age, reducing structural support

Genetics, sun exposure, weight changes and smoking all speed up this process. Jowls often appear in the late 30s to 40s and become more noticeable over time. They are frequently linked with marionette lines and sagging cheeks .

How a dermatologist assesses your jowls

Not all jowls are the same, and the right treatment depends on which layer is the main problem. During your consultation, the doctor looks at your face from the front, side and at an angle, in relaxed and smiling positions, and may gently lift the skin to see how it responds.

Key questions include: Is the main issue loose skin, deep SMAS laxity, heavy fat along the jaw, or volume loss in the cheeks above? How thick is your skin? How strong is the jaw muscle? The answers determine whether you need deep lifting, surface tightening, contouring or a combination. This assessment is the core of our Design Lift approach.

Treatment options, ranked by suitability

  1. Ultherapy Prime: micro-focused ultrasound reaching the SMAS; usually the first choice for mild to moderate jowls caused by deep laxity
  2. Ultherapy + Thermage combination: deep lift plus surface tightening for jowls with both SMAS and skin laxity
  3. Thermage FLX: monopolar RF for tightening loose, thinner skin along the jawline
  4. Fat-reducing and contouring options: such as Onda when heaviness under the jaw contributes
  5. Volume restoration: carefully placed filler or Sculptra in the cheeks or chin to support the jawline

Very advanced sagging may need surgical options; the doctor will tell you honestly if that is the case.

Home care and prevention

Home care cannot reverse established jowls, but it can protect your skin and slow further change. Daily broad-spectrum sunscreen protects collagen from UV damage. A retinoid or retinal product, used as tolerated, supports collagen renewal over time. A moisturizer with peptides or ceramides helps skin feel firmer and more resilient.

Stable body weight, not smoking and good sleep all help. Facial massage tools may temporarily reduce puffiness but do not lift the SMAS. Our preventive anti-aging page explains early steps for younger patients. Protecting results after treatment matters just as much as the treatment itself.

When to see a doctor, and planning around travel

See a dermatologist when jowls bother you, when your jawline starts to look less defined in photos, or before starting any lifting treatment, so you choose the right one. Seek medical advice promptly if sagging appears suddenly on one side, especially with weakness or numbness, as this can signal a nerve or medical problem rather than aging.

Most jowl treatments at The Heal have little downtime, so you can usually fly the next day. Lifting builds over two to three months after you return home. To get a personalized plan, send front and side photos with your travel dates , and read our lifting treatment trip guide.

The Heal approach

We identify which layer is causing your jowls, skin, SMAS, fat or volume, and treat that layer first rather than applying the same device to every jawline.

Frequently asked

Sagging Jowls: your questions answered

  • Can jowls be lifted without surgery?

Mild to moderate jowls often improve with non-surgical lifting such as Ultherapy Prime and Thermage FLX. Very advanced sagging may need surgery for a dramatic change. Results vary with age and skin quality.

  • At what age do jowls usually appear?

Many people notice early jowling in their late 30s or 40s, though genetics, weight changes and sun exposure can make it appear earlier or later.

  • Do fillers help with jowls?

Fillers do not lift jowls directly, but carefully placed filler in the cheeks or chin can restore support and make the jawline look more defined. They are often combined with lifting devices.

  • How long do non-surgical jowl results last?

Results from ultrasound or RF lifting typically last around a year or more, depending on age and lifestyle. Many patients schedule yearly maintenance to keep the jawline defined.

  • Can I fly home right after jowl treatment?

Usually yes. Ultherapy and Thermage have little to no downtime, so most people fly the next day. Mild swelling or tenderness may last a few days.
