Under-Eye Hollows Treatment in Seoul, Korea

Quick answer

Under-eye hollows, or tear troughs, are usually improved with a small, carefully placed amount of soft hyaluronic acid filler, often combined with boosters that thicken thin lower-lid skin. At The Heal in Jamsil, Seoul, a board-certified dermatologist checks whether filler is right for your anatomy first. Mild swelling or bruising may last a few days, and results vary. Send photos on KakaoTalk so we can advise before you book flights.

What causes hollows under the eyes?

The tear trough is the groove that runs from the inner corner of the eye down toward the cheek. When it deepens, it casts a shadow that makes you look tired even after a good night's sleep. Common causes include:

  • Genetics: some people have a deep trough from their 20s.
  • Fat and bone changes: cheek fat descends and the orbital rim recedes with age.
  • Thin lower-lid skin: blood vessels and muscle show through, deepening the shadow.
  • Weight loss: losing facial fat can reveal hollows quickly.

Hollows are often confused with dark circles or eye bags, which need different treatment.

How a dermatologist decides if filler is right

The under-eye area is one of the most technique-sensitive zones in aesthetic medicine. Dr. Yang examines you under overhead light to see where shadows fall, gently checks lower-lid skin thickness and elasticity, and looks for puffy fat bags, fluid retention or allergic swelling. Filler can make bags and puffiness worse, so if these are present she may suggest a different approach. She also asks about previous filler, since product lingering under the eye can cause swelling or a bluish tint. Only when anatomy and skin quality suit it does she recommend tear trough filler.

Treatment options at The Heal, ranked by suitability

  1. Soft filler, placed conservatively: under-eye filler using a low-density hyaluronic acid product, often with a cannula, softens the groove.
  2. Skin quality boosters: Rejuran Eye thickens thin, crepey lower-lid skin and is often combined with or used instead of filler.
  3. Mid-cheek support: restoring cheek volume can reduce the hollow from below.
  4. Gradual collagen: Juvelook may suit patients who prefer slow, subtle change.

Less product is almost always better here. Many patients see the most natural result from a booster first, with a small top-up of filler only if a groove remains after skin quality improves. Sessions are typically spaced two to four weeks apart.

Home care for the under-eye area

Use a gentle eye cream morning and night to keep thin skin supple, and apply sunscreen carefully around the orbit. Sleep with your head slightly elevated if you wake with puffiness, and limit salty food and alcohol in the evening. Treat hay fever or eye allergies, since rubbing and swelling make hollows look deeper. Concealer with a peach tone can offset shadows on the day. Caffeine eye products may temporarily reduce puffiness but will not fill a hollow, so expectations should stay realistic. Gentle makeup removal without tugging also protects this delicate skin.

Safety, downtime and when to see a doctor

Tear trough filler is safe in experienced hands, but bruising, swelling and occasional lumpiness can occur, and rare vascular complications are the reason only physicians should perform it. Hyaluronic acid filler can be dissolved if needed. Avoid treatment during pregnancy, with eye infections, or right before important photos. For international visitors, the main objection is bruising before a flight: allow three to seven days if possible, and read are dermal fillers safe? for more. Book an English consultation early in your trip so a follow-up check fits before you fly home.

The Heal approach

We assess bags, skin thickness and previous filler before choosing under-eye treatment, and we use less product rather than more.

Frequently asked

Under-Eye Hollows: your questions answered

  • How long does under-eye filler last?

Soft hyaluronic acid filler under the eyes commonly lasts around nine to twelve months or longer, because this area moves relatively little. Results vary with product, metabolism and skin quality.

  • Will I bruise after tear trough filler?

Bruising is possible because the area has many small vessels. Using a cannula and gentle technique reduces the risk, and most bruises fade within a week. Avoid blood-thinning supplements beforehand if your doctor agrees.

  • Can under-eye filler be removed?

Yes. Hyaluronic acid filler can be dissolved with an enzyme called hyaluronidase if you are unhappy with the result or experience prolonged swelling. This is one reason we use HA products in this area.

  • What if I am not a candidate for filler?

If you have prominent bags or very thin skin, boosters such as Rejuran Eye, skin tightening or a surgical opinion may suit you better. Dr. Yang will explain the options honestly.
